Motivational Horizons

Our Community-Based Adult Services Program

Motivational Horizons is a program of the Kern County Brain Injury Foundation dedicated to supporting older adults and adults living with chronic medical, cognitive, or behavioral health conditions. Through the CalAIM initiative, we provide Day Habilitation services covered by Medi-Cal, helping individuals build skills, engage in meaningful activities, and strengthen their independence in a supportive community setting.

Motivational Horizons provides Day Habilitation services to older adults and adults with chronic medical, cognitive, or behavioral health conditions who are experiencing or at risk of homelessness. Our program is designed to support individuals who need structured skill development and community connection to achieve greater independence and housing stability.


We serve:

  • Individuals currently experiencing homelessness who need structure, skill development, and social support
  • Adults who have recently exited homelessness (within the last 24 months) and are working to maintain housing stability
  • Individuals at risk of homelessness or institutionalization who could benefit from comprehensive day programming
  • Adults with traumatic brain injuries or cognitive impairments affecting daily functioning
  • Individuals with serious mental illness or behavioral health conditions requiring structured therapeutic support
  • Adults with substance use disorders working toward recovery and independence
  • Older adults who need social engagement, skill reinforcement, and community integration
  • Individuals with developmental or intellectual disabilities who can benefit from continued life skills education


Motivational Horizons Day Habilitation services are delivered at our facility in Bakersfield, California. Our welcoming, fully accessible center provides a safe and therapeutic environment where members can learn, practice skills, and build meaningful connections with peers and staff.


Program Hours: Monday – Friday, 9:00 AM – 5:00 PM


Beyond our facility walls, we extend learning into the community through supervised outings and real-world practice opportunities. Members participate in community integration activities throughout Kern County, applying the skills they've learned in authentic settings such as grocery stores, public transportation, community centers, and local businesses. This combination of structured facility-based programming and hands-on community practice ensures that skills transfer from the classroom to everyday life.


Our facility features adaptive equipment, comfortable gathering spaces for group activities, areas for individual skill practice, and all the resources needed to support comprehensive day programming. Transportation assistance is available for members who need support getting to and from the program.
